If you only have one free day outside Medellín, pick the trip that adds something your city days cannot. The useful way to approach this is to compare the parts of the day that actually affect you: transport, timing, group size, walking, inclusions and how much flexibility you want.
Do not use a day trip to repeat the city
Your Medellín days already give you neighborhoods, food, museums and urban viewpoints. A good first day trip should change the landscape or context: a reservoir town, a working coffee farm, colonial architecture or a mountain activity.
Guatapé is the broadest sampler
Guatapé combines a photogenic town with dramatic reservoir scenery and a very different pace from Medellín. It is also one of the easiest places to find transport-included tours, which is why it works well for first-time visitors who do not want to research every transfer.
Coffee is the deeper cultural option
A coffee-farm visit can be more intimate and educational than a multi-stop sightseeing day. If coffee is one of the reasons Colombia interested you in the first place, this can be the more memorable choice.
Leave some margin in the itinerary
Do not schedule a long rural excursion on the same day as an important evening reservation or flight. Road conditions, weather and group timing can change. A good day trip feels much better when the return time is not a source of stress.
